Head Start Program Details, hours, photos, information: The Learning Community (formerly Fenton Lawn)

To Apply for Head Start or Early Head Start you will need to complete and submitthe Learning Community Contact Form and provide us copies of: * Proof of residency. We will accept a Consumer's Bill, mortgage statement or lease agreement with your name and address to show you live in the Carman-Ainsworth School District. * Proof of income. We will accept your most recent W2, your income tax form, or DHS statement. * Birth Certificate. We must have a copy of your child's Official Birth Certificate. * Social Security Card. You must provide your child's social security card or statement that you have applied for one. * Medical Insurance Card. You must provide you child's insurance card. * Immunization Record. Must be current. * Physical Exam. This physical must be less than a year old and must include all necessary blood work. * DHS Case Number if applicable.
