To begin the process of enrolling your child in Early Head Start, Head Start or ECEAP, you must complete the application form, and submit income and age proof with the application.
The special consideration form is optional.
It is used to identify additional factors, which might qualify your child for the program.
Any information they are given is kept in strict confidence.
