Requirements for RegistrationThe following verification must be turned in before your child can begin:

• Birth Certificate
• Social Security card
• Certificate Degree Of Indian Blood (CDIB)
• Current Physical Exam
• Current Dental Exam
• Current Immunization Form 121
• Current PPD (TB Skin Test)
• Current Nutrition Assessment

It is better to turn in an application as soon as possible.

There is a wait list for entry into the center.
