    Necessary Documentation for Enrollment:
    -Certified Birth Certificate(Early Head Start: 6 weeks - 3 years of age; expectant families &Head Start: 3 - 4 years of age; 5 years of age after September 1)
    -Documentation of Income (Federal tax return,W-2 form,Statement of Income,Letter from Social Security Administration &Statement from the Department of Employment Security if zero income or unemployed)
    -Current Physical Examination
