ENROLLMENT DOCUMENT CHECKLIST ~ Proof of Child's Physical and Dental Examinations with Blood Work, Lead and TB Screening Results Included. Screening and Blood Work Must Be Dated No Earlier Than 90 Days Before Date of Enrollment. Proof of Enrolling Parent or Primary Caregiver's Recent TB Screening with Results Copy of Child's Birth Certificate Copy of Enrolling Parent or Primary Caregiver's Driver's License or State ID Most Recent Proof of Income for Enrolling Parent or Primary Caregiver - includes but is not limited to Payroll Check Stub, Unemployment Benefits Letter, TANF Award Letter, SSI/SS Benefits Letter Information and Photo IDs of at least 2 Emergency Contacts Medical Card, if applicable
